As a 501(c)3 nonprofit clinic we give uninsured individuals who live at or below 200% of the Federal Poverty Guidelines hope for a healthier life. We provide medical treatment & holistic care without charge.

Our clinic offers the following services:

Primary medical care for adults with chronic illnesses

Basic primary care for adult patients suffering from chronic medical conditions like diabetes, hypertension, seizure disorders, thyroid conditions, asthma, COPD, lupus, etc.
Free prescription medication for patients with chronic conditions

Dental care - temporarily on hold

Basic dental extractions and minor fillings

Accessible general health services

Speciality Care Referrals
Women’s Health

If you need healthcare beyond what we can provide, we have a network of specialty care referrals

OBGYN services and breast health care, including screening mammograms and diagnostic services

Holistic health & lifestyle education

Chronic Care Education

Chronic care education uses a team care approach to combine quality medical care with coaching and education to equip patients with the skills to improve their overall health and inspire self-care.

Eligibility requirements:

We don’t just focus on treatments. We believe everyone deserves to be healthy, supporting others is the work worth doing, and prevention is better than just finding a cure.

Aged 18 or older
A resident of Bulloch County
Medically Uninsured (no private insurance and ineligible for Medicaid and Medicare)
Live at or below 200% of the Federal Poverty Guidelines
Have a diagnosed chronic disease (diabetes, hypertension, seizure disorders, thyroid conditions, asthma, COPD, etc.)

Each patient must complete an in-person application and provide the following documents:

Proof of residency in Bulloch County (driver's license, ID Card, utility bill)
Proof of family income (pay stubs, income statements, W-2, tax returns)
Medicaid denial letter (this is required to receive free medication; we can help you apply for Medicaid)
